So Neil defeated the Balrog and was reborn as Neil the White
Either Nail is damn near unkillable (as far as I can recall, the last time we saw him he was soundly impaled by those spider heart thingies) or this is a Nightmare in disguise...

There's always the option that Nail was always a nightmare. He's got a bit of the psycho slasher vibe.
Nail is a ghoul, isn't he? My headcanon is that Veda somehow made some kind of deal with Nail to work for her in exchange for something or whatever.
I want to say "Cool, A Nail in a coughing!", but that would be dishonest, given that it was more of a blood-vomit than a cough...
The strangest thing to me is that Nail found his way here without Noah.
